Position Responsibilities
In addition to the required qualifications, a successful Patient Navigator will work directly with patients to provide referral navigation, support and scheduling the logistical aspects of the patient’s treatment and care. Enable closed loop referrals with PCP’s and build trust in the system for all parties.
