GENERAL JOB DESCRIPTION

OVERVIEW: Mount Saint Vincent (MSV) in Wellesley, Massachusetts is a retirement community for the Sisters of Charity-Halifax who live in the United States. There are two facilities on the MSV campus: the Elizabeth Seton Residence (a skilled nursing and rehabilitation facility) and Marillac Residence (an assisted living facility). Care Coordinators work collaboratively with the staff of the two facilities to provide comprehensive support and advocacy for approximately 75 retired women religious. The Care Team reports to Congregational Leadership and is responsible for being attentive to the health and well-being of the retired Sisters of Charity in the MSV community.

ESSENTIAL DUTIES AND RESPONSIBILITES:

To be present and accessible on-site at Marillac Residence and Elizabeth Seton Residence five days per week, Monday through Friday.

To actively engage with Sisters, build relationships and provide care and support.

To be attentive and responsive to the needs of the Sisters, and serve as an advocate for their health and well-being.

To drive and accompany Sisters to medical appointments and other healthcare settings as needed.

To build relationships, foster collaboration and communication, and enhance community with other staff who work at Mount Saint Vincent.

To work a schedule that meets the needs of the Sisters. This includes being part of a team that shares scheduled on-call coverage of evenings, weekends, and holidays.

To perform other duties and activities of greater, equal or lesser responsibility as required.

QUALIFICATIONS FOR THE POSITION:

EDUCATION: Bachelor’s Degree with professional experience working with older adults; preferably familiar with memory and end-of-life issues.

OTHER: Valid Driver’s License and physical capacity to assist Sisters as part of accompaniment and transportation responsibilities.
