General Summary: The Patient Care Navigator is responsible for assisting nurses and social workers to provide outreach efforts to a group of patients from a patient list. This role will utilize the established model of care management. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures, including complying with all Rush University Medical Center Customer Service Standards.
Required Qualifications Include:
High School Diploma or equivalent.
1-3 years’ experience of EMR required
Working knowledge of the needs of the LGBTQ community and relevant local resources.
Must be comfortable with outreach activities in unstructured environment
Must be prepared to work occasional offsite events.
Ability to work with culturally, racially, and ethnically diverse clients and staff.
Excellent interpersonal and organizational skills
Preferred Qualifications Include:
Personal experience navigating the healthcare system
Fluent in Spanish
Principal Duties and Responsibilities:
Completes intake for new patients and provides appropriate referrals to GSR services, care at Rush, and community resources.
Provide outreach for all patients due for follow up and new patient appointments according to policy and procedures.
Prepare and deliver routine correspondence/medical records to referring agencies and primary care providers in accordance with policy.
Completes all EHR documentation in a timely manner.
Communicates with the Manager of Social Work about individual client’s needs and concerns.
Monitor compliance with scheduled appointments for procedures, tests and consults.
Document all correspondence in electronic medical records as necessary.
Work to improve identified outcomes (i.e. high utilizer patients) by using quality measures.
Work with care team to identify extrinsic barriers to adherence (i.e. transportation, financial concerns or pharmacy) and work with patient and team to alleviate barriers.
Liaison to multiple departments, agencies, and provider offices.
Participates in all administrative, staff development and staff meetings.
