Position Summary
As a Care Management Consultant, you are responsible to provide a holistic, client-centered approach to caring for clients. The Care Management Associate provides assessments and continuous reassessment and comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence and wishes are respected.
Minimum qualifications
- Three years of experience in community, hospital, hospice or home health care management
- Certified Care Manager (CMC) or Certified Case Manager (CCM)
- Demonstrated knowledge of aging, dementia, and/or disabilities.
- Experience working with complicated family systems and dynamics.
- Excellent computer skills to include MS Office, Electronic Health Records and Internet Skills
Preferred
- Bachelor’s degree in Healthcare Related field

Compensation range:
$60-$85/hr.
