Holy Cross Hospital in Fort Lauderdale, Florida is a full-service, non-profit Catholic hospital, sponsored by the Sisters of Mercy and a member of Trinity Health.
Develops and maintains relationships with all targeted health systems, hospitals, physician/at-risk organizations and post-acute providers (home health agencies, skilled nursing facilities, inpatient rehabilitation facilities, long term acute care hospitals).
Supports execution of Post-Acute Care network evaluation and performance optimization strategy through facility tours and survey and implementation of SNF Care Guidelines; continuously monitors market trends and opportunities for network evolution and expansion.
Monitors key performance metrics related to the post-acute network to proactively address unfavorable clinical and/or operational trends and develop action plans to positively
influence performance and meets regularly with health care providers to review performance.
Facilitates network meetings with agenda, minutes and outcome data and other standing meetings with post-acute providers to communicate strategy and review/improve performance.
Maintains patient-facing material to support choice and preference while incorporating education on high performing PAC providers.
Responsible for driving and supporting NSOC (Next Site of Care) strategies and post-acute care coordination programs by promoting a collaborative, open, and inclusive work environment within a highly matrixed organization.
Collaborates with healthcare professionals across healthcare settings to promote quality patient outcomes and PAC placement at the clinically appropriate, least restrictive next site of care.
Works with internal departments to generate ideas and develop and prioritize joint strategies for business and/or technology enhancements that will support patient outcomes, providers and attainment of financial performance metrics.
Performs other duties as assigned.
MINIMUM QUALIFICATIONS:
Graduation from accredited Physical Therapy program, Occupational Therapy program or Master’s degree in Speech Language Pathology, current associated licensure in the state where working with 5 years of clinical and/or operational experience; 3 years of which are in acute care and/or skilled nursing facility, home health or other post-acute environment.
Previous business relationship development and management experience with hospitals, SNFs, ACOs and/or home health agencies. Population health, hospital/post-acute case management and/or clinical background a plus.
Ability to manage multiple tasks, be detail oriented, be responsive, and demonstrate independent and innovative thought and critical thinking.
Knowledge of acute and post-acute industries, ACOs, risk/value based arrangements and care delivery trends is preferred or a willingness to learn.
Knowledge and understanding of post-acute admissions processes and acute and post-acute discharge planning process. Working knowledge of managed care and/or Medicare Advantage.
Understanding of PAC reimbursement structures/value based payment models or willingness to learn.
Proficiency in Microsoft Excel, PowerPoint, Word.
Excellent communication (verbal and written) and presentation skills.
Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the mission, vision, values and goals of Trinity Health.
