Responsibilities
Position Summary:
The Diabetes Nurse Educator functions as a caregiver for patients with diabetes in order to achieve desired outcomes. The purpose of this role is to improve diabetes care to comply the regulatory performance measures for quality care. This position directs and oversees the care provided by other caregivers through education, delegation, validation, evaluation and coordination. This position effectively coordinates patient care activities of the patient with diabetes among nursing peers, physicians and support services.
Qualifications
Position Qualifications:
• M.S. Degree in Advanced Practice Nursing
• Prior work experience in a healthcare environment.
• Certification in continuing professional education of diabetes self-management education and support
• Experience in Subcutaneous Continuous Insulin Infusion (insulin pumps)- education for initiation, titration, and trouble shooting.
• Experience with continuous glucose monitoring
• Experience with treatment and management of Type1, Type2 and gestational Diabetes.
• Must hold a valid License as a registered Nurse in the state of NJ
• Certification in continuing professional education of diabetes self-management education and support
• Board Certified Diabetic Educator by the
National Certification Board of Diabetic Educators(NCDBE)
Certified as an Adult Nurse Practitioner by the American Nurses Credentialing Center (ANCC)
• Excellent attention to detail.
• Excellent verbal and written communication skills
• Must be willing to provide coverage of the center on some off hours to include at a minimum of one evening a week and/or Saturday
