Home Health Registered Nurse (RN)

We are looking for a compassionate and experienced Home Health Registered Nurse to join the team! This position is responsible for providing basic assessments to patients in their homes.

Responsibilities:

• Conduct initial and ongoing assessments of patients’ physical and mental health needs

• Develop and implement individualized care plans

• Educate patients and their families about self-care and disease management

• Collaborate with other healthcare professionals to ensure that patients receive the best possible care

• Document all care provided in accordance with agency policies and procedures

Qualifications:

• Current and valid RN license in the state of GA

• At least two year of experience in home health nursing

• Strong assessment and reporting skills

• Excellent communication and interpersonal skills

• Ability to work independently and as part of a team
