MLTCP – Care Manager
The Care Manager plays a critical role in the management of clinical care and resource utilization for all assigned clients. He or she is responsible for the care management of assigned clients, to include home visits as per regulation or as necessary. S/he interfaces with service providers, caregivers, families and clients to ensure clients are able to remain safely in their homes as long as possible and within the guidelines of a Managed Long Term Care Program. Must be able to interface well with professionals within and outside of the organization, as well as with potential clients and their families. Familiarity with the clinical issues faced by MLTCP clients and the appropriate management of such is critical. Time management and organization skills are essential in this volume-driven, fast-paced environment. A love of working with the elderly and disabled must be evidenced in all interactions, at all times. Must be able to make decisions that are financially prudent, highly ethical, in compliance with all governing regulations, and demonstrate a commitment to our client’s welfare and well-being.
The following educational requirements apply to this position:
Required: Associate’s Degree in Nursing, Social Work or Related Field.
Preferred: Bachelor’s Degree in Nursing, Social Work or Related Field
The following experience requirements apply to this position:
At least 5 years of professional nursing, social work or related field experience; experience must include clinical and functional assessments
Previous healthcare experience
Previous long term, home care or MLTCP experience
