What we offer:
About you:
You will use your health care experience, talents, and problem-solving skills to develop individualized comprehensive care plans using doctor’s notes and patient chart data to help patients manage their health between office visits. You will use our best-in-class Care Management software to support patients on a variety of programs, including Chronic Care Management, Behavioral Health Integration, Remote Patient Monitoring, and more. This is a knowledge-based opportunity where you will leverage your clinical experience, not a directly technical role other than developing specific expertise with our care management software application.
