Heritage Village of Clyde, a Certus Healthcare Management Community, is seeking an Admissions Director to join our team!

We are looking for someone with:

• The ability to manage inquiries, admission, and re-admission from the hospital, community, and other referral sources
• Experience in admissions in long term care.
• Experience in generating quality census mix
• Who can provide a high level of customer services to patients/residents/families/referral sources to ensure an efficient referral and admission process.
• Experience in Tracking referrals, inquiry’s, and patient/resident data to ensuring accurate, thorough, and timely entry of data into various database systems
• Knowledge of Verifying insurance benefits and coordinates financial and clinical approval of referrals, communicating outcomes with appropriate departments.

This position is ideal for someone with prior admissions experience in long term care or a business office manager looking to grow into an admissions role. We are looking for this person to assist in converting hospital or community referrals into short term or long term stays within our community and overall assist creating a positive environment with the interdisciplinary team, in the community.
