Responsibilities
Continuous Care Coordinator Responsibilities
- Triage all inquiries requesting continuous care.
- Work with the Case Manager or Manager, Clinical Team to evaluate the patient’s appropriateness for continuous care.
- Responsible for coordinating with the Case Manager and/or Manager, Clinical Team to their re-evaluation for appropriateness to continue continuous care after the initial period.
- Documents all conversations on the proper form both paper and in the Electronic Medical Record (EMR) and brings/sends to hospice office on a weekly basis.
- Keep updated daily reports to all staff regarding the status of the patients on cc via email.
- Assist with visits as needed per approval from the Director of Clinical services.
Qualifications
Continuous Care Coordinator Qualifications
- Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) or Registered Nurse currently licensed in the state(s) practicing preferred. Can be a non-nurse if the administrative team agrees on the candidates’ qualifications.
- A minimum of one year of hospice experience preferred.
- A minimum of one year of supervisory, continuous care coordination, scheduling or triage experience.
- Demonstrated leadership and organizational skills.
- A valid driver’s license and minimum state required liability auto insurance.
